For your convenience, please print and fill out all applicable forms and present them to the front desk when you arrive for your first office visit.

Insurance Verification

Our phone system is open from 8:30 AM to 5 PM Monday through Friday. The prompts will help direct your call to the correct department to handle your specific needs. If your physician is with a patient he/she may call you at a later time that day. Our goal is to call you within 24 hours. Please let us know if this is an emergency and cannot wait 24 hours. If you should have an emergency during the time our office is closed, please dial the office number and the answering service will direct your problem to the “on-call” doctor who will meet you in the ER if necessary. We are affiliated St. Luke’s Cornwall Hospital.

Patients are normally seen by appointment. If you arrive late for your appointment, it will be at the discretion of the physician whether or not you will be seen. Should your medical and/or insurance information change, it is your responsibility to inform the receptionist. Patients are seen by scheduled appointment time. If you arrive early, expect that those who have appointments before you will be seen first, even if they arrive after you. We apologize and ask for your patience when we fall behind schedule. Emergencies do occur and we do our best to accommodate all our patients. You may call ahead to see if we are running on time and we will be flexible in rescheduling you if requested. Please notify us as soon as possible if you cannot make your appointment, so we can accommodate another patient. Unaccompanied minors (anyone under 18) need to bring authorization from a parent or legal guardian to receive medical care.

Prescriptions are issued during office hours only as the physician must have access to your medical record. Please try to anticipate your need for refills. When requesting a prescription or a refill, it will help if you have your medication information and pharmacist’s phone number available when you call.

For copies of your x-rays, please call us 24 hours in advance so that we have time to prepare them. There will be a minimal fee for copies.

We welcome questions regarding fees and billing statements. Some services are billed globally. For instance, a fracture or surgery charge usually includes follow-up visits within 30-90 days of the procedure. This does not include x-rays and supplies. After this period, office visits will be charged.

Frequently, the explanation of benefits from your insurance company will call fracture care ‘surgery, ’ even though surgery has not been performed.

We ask that you read carefully the Financial Policy statement you will be given to sign and discuss any questions with our billing office prior to your appointment.
